From July 1st 2025, Medicare is increasing the eligibility for what it calls Chronic Condition Management Plans (CCMP) to anyone with a chronic condition. This includes diabetes, arthritis, osteoporosis, heart disease, dementia, obesity, sleep apnoea, high blood pressure and more.
This allows health goals to be set between yourself and your medical team, which can be reviewed up to every 3 months.
Patients with a CCMP are eligible to be referred for up to 5 medicare rebatable allied health visits (eg. podiatry, physiotherapy, dietician, exercise physiology, etc). This is worth about $300 of rebates in total per year.
These plans are the best way to ensure you’re staying on top of your health goals and preventing any health complications as best as you can.
